  • Wilmington residents age 14 and older may fill out the online application below. 
  • If you are not a Wilmington resident, please read the Circulation Policy to see if you are eligible for a card.
  • Wilmington residents under the age of 14 need parental/guardian permission and must print and fill out this form. The completed form can then be brought to the library (if open) or it can be scanned and emailed to kids@wilmlibrary.org or faxed to 978-658-9699, in which case the library card will be mailed to you. 

If you already have a card but can’t remember your PIN (or have other questions about your account), please email us at circulation@wilmlibrary.org or kids@wilmlibrary.org.  Library card passwords/pins are set to the last four digits of your phone number.
